Patient Financial Responsibility

  • Please confirm with your insurance provider that Diabetic Care Associates and our providers are in-network prior to your first appointment. If you are seen and our practice is not recognized by your insurer, you will be financially responsible for the visit.
  • Your insurance agreement determines your financial obligations, including copays, deductibles, and out-of-pocket limits. It is Diabetic Care Associates responsibility to bill and collect payment from you directly for the portion of your bill that your insurance does not cover.
  • All coverage questions should be directed to your insurance company.
  • Services deemed non-covered or out-of-network by your insurance will be your responsibility.

Missed Appointments/No-Shows/Late Cancellations/Late Arrivals

  • Please inform us 24 hours in advance of your appointment if you need to cancel or reschedule whenever possible. If you miss, cancel, or reschedule an appointment without proper notice it will be recorded as a “no-show” or “late-cancel.”
  • A fee of $40 may be assessed for a “no-show” without proper cancellation.  A fee of $80 may be assessed if you are a new patient and “no-show” or “late-cancel.”
  • If you arrive more than 10 minutes late, we may need to reschedule your appointment.

Missed Copay

  • We are required to collect copayments at the time of your appointment.
  • We can’t waive this payment due to the contract agreement with your insurance company. If you are unable to pay your copay at the time of your appointment, we may charge a missed/late copay fee of $15.

Insufficient Funds/Returned Checks

  • If we are notified by your bank that there are insufficient funds in your account to cover your payment, you will be charged a $35 fee.

Account Balances

  • We expect all patients to pay their bills in full within 30 days of receiving a statement from us. Please get in touch with our office to set up an arrangement if you need more time to pay the balance.
  • 60 Days: Finance charges will begin to accrue.
  • 90 Days: Not allowed to schedule appointments until balance is paid.
  • Failure to pay an overdue balance or make arrangements for payment on your account is cause for dismissal from the practice.
